Record Chronology and Evidence Matrix

Build a chronology that keeps occurrence, entry and availability apart; test an opinion against the evidence that cuts against it; and request missing records by the question each can answer. Companion to the guide Build a Medical-Record Review That Can Survive Contrary Evidence.

Your entries stay on this device. This page works entirely in your browser. Nothing you type is sent to a server or saved after you close or reload the page. To keep a copy, use Print or save as PDF. Use fictional or de-identified information only. Do not type patient names, dates of birth, record numbers or other identifiers here. Real case material belongs in the engagement process on the inquiry page, after conflict screening.

What this tool does and does not do

Does: holds a chronology with separate time columns, status labels (Documented / Inferred / Unknown), an opinion-and-evidence matrix with the same rows as the site's published Sample 2, and a missing-records list keyed to the question each record answers. Flags rows missing a status or source.

Does not: read or import records, calculate deadlines, judge negligence or causation, interpret audit logs, or store anything. Interpreting audit-log events and EHR display behaviour may require an EHR technical expert; causation usually requires a specialist in the relevant field.
