After a Test, Referral or Medicine Change: Follow-Through Organizer

Write down what was done, what you were told, who is responsible for the next step and when it should happen. Then keep a short log of every call or message until it is done. Print it or save it as a PDF to bring to your next visit.

Your entries stay on this device. This page works entirely in your browser. Nothing you type is sent to a server or saved after you close or reload the page. To keep a copy, use Print or save as PDF. You may enter your own health information, but it is never transmitted. If you share a device, clear the page when you finish.

What this tool does and does not do

Does: keeps one result, referral or medicine change organized: what was done, what you were told, who owns the next step, the expected date, how you will be told, every contact attempt, and your questions. Highlights a follow-up date that has passed without being marked done.

Does not: interpret results, tell you whether something is urgent, book appointments, contact anyone for you, or decide insurance or legal questions. If you have new or worsening symptoms, contact your care team or seek urgent care. In an emergency, call 911. For a mental-health crisis, call or text 988.
