Abnormal-Result Responsibility Worksheet

Map every step of one result or referral process to a named role, a backup, the access and authority the role needs, what happens when a step fails, and what record proves the step was done. Then test it with a tabletop exercise.

Use role titles, not names, and never enter patient information. This worksheet maps a process, not a patient.

What this tool does and does not do

Does: lays out the ten steps of a result or referral loop with an owner, a backup, required information, access, authority, an organization-defined time standard, an escalation route, completion evidence and after-hours coverage. Highlights steps with no owner, backup or completion evidence. Includes a tabletop log.

Does not: supply clinical urgency thresholds or turnaround times. Those must come from your organization's current, clinically reviewed policy and the applicable condition-specific guidance. It does not certify compliance with any accreditation standard or regulation.
