Fatty Liver Disease: Understanding a Common and Reversible Condition

Fatty liver disease is increasingly common and closely tied to metabolic health. The encouraging news is that it is often preventable and reversible with lifestyle changes.

Fatty liver disease, now often called metabolic dysfunction-associated steatotic liver disease, occurs when excess fat accumulates in the liver, most commonly related to obesity, insulin resistance, type 2 diabetes, and elevated triglycerides — the same cluster of factors associated with cardiometabolic risk generally. It has become increasingly common as rates of obesity and metabolic syndrome have risen.

The encouraging aspect of fatty liver disease is that, particularly in its earlier stages, it is often preventable and reversible through the same lifestyle changes that improve overall metabolic health: modest weight loss, improved diet, and regular physical activity.

How Fatty Liver Disease Develops

Fatty liver disease develops when the liver accumulates more fat than it can efficiently process, closely linked to insulin resistance — the same underlying process involved in prediabetes and type 2 diabetes. Obesity, particularly excess abdominal fat, elevated triglycerides, and metabolic syndrome all significantly increase risk.

Most people with fatty liver disease have no symptoms at all, and it is often discovered incidentally through routine blood tests showing mildly elevated liver enzymes, or through imaging done for another reason. When symptoms do occur, they tend to be vague, such as mild fatigue or discomfort in the upper right abdomen.

Why It Matters

In most people, fatty liver disease remains relatively stable and does not cause significant liver damage. However, in a meaningful subset of people, it can progress to inflammation (steatohepatitis) and eventually scarring (fibrosis or cirrhosis) of the liver, which can lead to serious liver disease over years. Identifying and addressing fatty liver disease early reduces this risk.

Fatty liver disease is also considered a marker of broader cardiometabolic risk — people with fatty liver disease have a higher risk of cardiovascular disease and type 2 diabetes, making its identification a useful prompt to evaluate and address overall metabolic health, not just the liver specifically.

How It Is Diagnosed

Fatty liver disease is often first suspected based on mildly elevated liver enzymes on routine blood work, then typically confirmed with an ultrasound or other imaging showing fat in the liver. In some cases, additional testing, such as specialized imaging or a liver biopsy, may be recommended to assess whether inflammation or scarring has developed, particularly if initial findings suggest more advanced disease.

Your doctor will also evaluate for other causes of liver enzyme elevation or fat accumulation, such as significant alcohol use, certain medications, viral hepatitis, or less common liver conditions, since the workup and treatment differ depending on the underlying cause.

Treatment Through Lifestyle Change

Weight loss of even 5-10% of body weight has been shown to meaningfully reduce liver fat and, in many cases, improve or resolve early-stage fatty liver disease. A diet emphasizing whole foods, reduced added sugars and refined carbohydrates, and limited saturated fat supports this process, alongside regular physical activity.

Limiting or eliminating alcohol, since it adds an additional burden on the liver, and managing related conditions like diabetes and high cholesterol as part of a comprehensive approach both support liver health. For most people with uncomplicated fatty liver disease, no medication is specifically approved, and lifestyle change remains the primary treatment. For a more advanced, defined subset — noncirrhotic MASH with moderate-to-advanced (F2–F3) fibrosis — the FDA has approved two medications, resmetirom (2024) and semaglutide (2025); whether either applies depends on liver-specific staging that only your doctor can confirm.
