California County Contract Atlas: Where CFMG and Wellpath Operate — and Where Counties Have Moved On
The CFMG–Wellpath relationship looks different when viewed from a County Board chamber than it does from a corporate chart.
California counties are the government clients. Their contracts specify what services must be delivered, how much the County will pay, what staffing is required, what oversight exists, and what happens when service levels are missed.
Those records also reveal something important about identity: the same healthcare operation may appear publicly as California Forensic Medical Group, Inc., CFMG dba Wellpath, CFMG/Wellpath, or simply Wellpath.
That nomenclature is evidence of operating integration.
It is not, by itself, proof that all of those names describe one legal entity.
This atlas therefore records the exact contracting language wherever possible.
The current public map
The table below is not yet an exhaustive list of all 58 California counties. It is the first verified public-contract layer.

The source record is already enough to reject two simplistic models.
Model 1: “Wellpath replaced CFMG everywhere”
The County contracts do not support that.
Fresno, Santa Barbara, El Dorado, Sonoma and other counties continue to identify CFMG in the formal agreement.
Model 2: “Wellpath is irrelevant because CFMG signs the contract”
That is also incomplete.
The same County materials repeatedly use Wellpath as the operating identity, and Wellpath supplies the broader management platform described in the public MSA.
The better model is:
CFMG often remains the formal California contracting professional corporation while Wellpath appears as the enterprise or management identity around the contract.
Alameda County: a large CFMG contract under a public procurement record
Alameda County’s awarded-bid database lists a Comprehensive Medical Services award to California Forensic Medical Group Inc. beginning October 1, 2022, with a listed amount of approximately $254.4 million.
That is strong public evidence of CFMG’s continuing role as a major government contractor after the Wellpath name became dominant.
Labor records from Santa Rita Jail use different nomenclature, including “Wellpath formerly California Forensic Medical Group.” That discrepancy belongs in the labor/employment analysis rather than being silently imported into the contract record.
El Dorado County: CFMG by exact legal name
In February 2025, the El Dorado County Board approved a retroactive agreement with California Forensic Medical Group, Incorporated for jail and juvenile-detention health services.
The agreement covers July 1, 2024 through December 31, 2029 and has a maximum of $45 million.
The County’s staff report emphasizes continuity of legally required correctional care.
Santa Barbara: a renewed CFMG relationship after public alternatives were considered
Santa Barbara County’s contracting history is particularly useful because it shows repeated public extensions followed by a new agreement.
On April 1, 2025, the Board approved an agreement with CFMG for medical and mental-health care at Sheriff custodial facilities and medical care for youth at Probation facilities. The two-year base term has a maximum of $49,768,838, with three one-year extension options.
Santa Barbara County File 25-00307
The County had publicly discussed alternatives, including the possibility of County Public Health delivering the services. That is important because it shows the contracting model is a policy choice, not an inevitable legal arrangement.
Santa Cruz: CFMG and Wellpath in the same budget sentence
Santa Cruz County’s 2024–25 budget describes the Corrections Bureau medical-services function as contracting with “California Forensic Medical Group (Wellpath)” for inmate medical, dental and mental-health services.
The proposed service expense was approximately $9.65 million.
By 2026–27, the Sheriff budget describes an approximately $12.0 million medical and behavioral-health contract.
The County’s Grand Jury has separately examined staffing and service performance. Those oversight findings matter for contract administration; they should not be transformed into corporate-ownership conclusions.
Sonoma: CFMG contract, Wellpath name, and separate Wellpath agreement
Sonoma County approved a 2021 agreement with California Forensic Medical Group, Inc. for inmate medical and dental services through October 2026, with extension options.
In 2026, a CalAIM-related amendment described CFMG as “commonly known as Wellpath” and increased the contract total to approximately $64.6 million. The same Board item separately authorized a medication-assisted-treatment agreement with Wellpath.
That is a useful example of why exact entity names must be recorded contract by contract.

Two counties demonstrate that CFMG can lose the contract
A serious analysis should include evidence contrary to any theory that counties are structurally captive.
Monterey
Monterey County had a long-running CFMG/Wellpath relationship.
A 2022 amendment identified the contract as California Forensic Medical Group, Inc. (CFMG dba Wellpath) and extended service through December 31, 2025.
But in 2025 the County conducted a new RFP and selected Correctional Healthcare Partners Monterey, Inc. The new contract began January 1, 2026, with a not-to-exceed amount of approximately $139.4 million through June 2031.
Monterey New Provider Contract
Placer
Placer County announced in May 2026 that it had selected Correctional Healthcare Partners after a competitive process. The new $131.28 million agreement began July 1, 2026.
The County expressly said CFMG would continue as the existing provider through June 30 during transition.
These transitions matter.
They show that the public contracting environment includes genuine competition and County choice.
What the atlas is designed to measure
As the atlas grows, every County entry should capture the same variables:
- exact contracting legal entity;
- use of Wellpath branding;
- contract value and term;
- medical-director requirements;
- staffing matrix;
- behavioral-health scope;
- dental;
- utilization/referral provisions;
- off-site care;
- pharmacy;
- records/EHR;
- quality-assurance obligations;
- insurance and indemnity;
- County audit and monitoring rights;
- staffing-credit or vacancy reimbursement;
- termination rights;
- subcontracting;
- CalAIM obligations;
- litigation/remedial-plan overlays.
Those are not merely contract details.
They determine where government-client authority ends and contractor authority begins.
The emerging statewide picture
The first verified public set shows three recurring patterns.
1. CFMG remains legally visible
Even years into the Wellpath era, counties continue signing contracts with CFMG.
2. Wellpath is often the operating name
County budgets, labor records and staff reports frequently append or substitute the Wellpath name.
3. County authority is substantial
The government client can:
- select a provider;
- replace it;
- impose staffing levels;
- structure reimbursement;
- require quality metrics;
- allocate off-site costs;
- approve subcontractors;
- and monitor performance.
That means the California authority map is not simply:
Wellpath → CFMG → clinicians
It is at least:
County/client ↔ CFMG professional contractor ↔ Wellpath management platform ↔ licensed clinicians
