New supplement: California’s Corporate-Practice Enforcement Record, 2021–2026 — Attorney General actions (Art Center amicus, Carbon Health, Aspen Dental, Orange Twist), nineteen Medical Board matters, Osteopathic Board posture, amici, Discovery Radiology and Mochi, and the AB 3129 → SB 351 + AB 1415 path, each with its procedural status and mapped to a twelve-indicator CFMG–Wellpath control matrix. Not a finding that the CFMG–Wellpath relationship violates California law.

County File 26-0855 now records Amendment XIII as Agreement No. 26-459 and Resolution No. 26-329, with a Digital Signature Certificate among the filed materials. The Legistar “Final action” field remains blank, so Board approval is not characterized as conclusively recorded from that page alone. The recommended action names California Forensic Medical Group, Incorporated as counterparty and raises cumulative maximum compensation by $6,104,438 to $400,479,492. A cumulative contractual ceiling accumulated through thirteen amendments — not an annual payment and not new money. The County's discussion says Wellpath will provide and coordinate the CalAIM Justice-Involved services, collaborate with WestCare California and the managed-care plans, and ensure that its staff are credentialed to bill Medi-Cal; it names Medusind, Inc. as Wellpath's subcontracted billing vendor, paid 7% of gross revenue the County collects. A stipulation and order in Estate of Jeremiah Wright v. County of Stanislaus identifies Medical Defense Law Group attorneys as counsel for CFMG, Wellpath Management, Inc. and individual medical defendants together, after the 15 September order had treated the entities separately for discharge. Defense-integration evidence only — not proof of common control or common employment. See: Article 057
- 20 September 2026 · Santa Ana
City Council agenda calls for a second amendment involving CFMG and Wellpath for healthcare at the Santa Ana Jail: a further one-year renewal through 30 September 2027 at $3,664,243 annually, bringing cumulative authorised compensation to no more than $17,507,799.

The Hernandez v. County of Monterey contempt settlement administrator reports first settlement payments mailed. The settlement site identifies CFMG as the jail healthcare provider and states CFMG agreed to pay $2.47 million regarding contempt liability for violations of healthcare-related court orders.
CFMG’s distinct post-bankruptcy liability has moved from approval into payment administration.

The Eastern District of California issues a discovery order in Estate of Jeremiah Wright v. County of Stanislaus, No. 2:24-cv-02505. The caption separately identifies CFMG, Wellpath LLC and Wellpath Management, Inc. alongside individual medical personnel. All three corporate layers appear simultaneously in one current California case.
The order concerns reconsideration of discovery rulings; it does not resolve the relationship among the entities. See: The Three-Entity Problem

Anderson v. County of Santa Barbara, C.D. Cal. No. 2:26-cv-10199, filed. The complaint separately names California Forensic Medical Group, Inc. and “Wellpath, Inc.” Another post-restructuring civil-rights case pleading the identities separately.
Defendants’ eventual Rule 7.1 disclosures may become useful evidence on the ownership question.

Wellpath announces a Local Government–California operating division led by Jessica Mazlum as Division President, and describes CFMG as a physician-owned professional corporation affiliated with Wellpath’s management-services organisation.

The National Labor Relations Board processes a representation case identifying California Forensic Medical Group, Inc. as the employer across a professional and a nonprofessional unit, and certifies the union after the election. The Beckner summary-judgment order names CFMG and its employee the “CFMG Defendants” while Wellpath Inc., Wellpath LLC and Wellpath Management, Inc. stand discharged — different bankruptcy consequences inside one case.

El Dorado County files an administrative-expense application in the Wellpath bankruptcy identifying “Contract #2989 … Between County and California Forensic Medical Group” and stating that the contract was assumed by the Debtors. What this page establishes

No entry here adjudicates the professional-control question. Contracting activity is not evidence of who holds authority over physician-reserved decisions, and neither is a docket caption.
